Enrolling Your Child

Registrations are accepted any time during the year provided space is available. Please come to the center to register your child.

A new registration form is required for each school year and each summer program. The school year program begins when the public school starts, approximately the 3rd week in August. The summer program begins when the public school year ends, approximately the last week in May.

Sullivan Preschool admits students of any race, sex, color, nationality, and ethnic origin to all the rights, privileges, programs, and activities generally accorded or made available to students at the school. It does not discriminate on the basis of race, sex, color, national or ethnic origin in administration of its educational policies, admissions policies, scholarships and loan programs, and athletic and other school-administered programs.

The parent or guardian will be required to fill out and sign the Application and Child’s Personal Record form, Emergency Medical Care consent form, consents for field trips off the premises, photographs for publicity purposes, participation in the swim program, the administering of medication and first aid, the school’s policy on guidance and discipline, and a contract for the payment of tuition which includes the child’s scheduled days and hours of care. Social Security numbers for the child and for at least one parent are required by the Illinois Department of Children and Family Services; this information is kept in the child’s confidential enrollment file.

Emergency telephone numbers must be provided for at least two persons who are available and willing to call for the child in case of an illness, accident, or other emergency in the event that the parent is not available. Parents taking classes at a college, university or enrolled in any other training program, must provide a copy of their class schedule and a means of notification in case of an emergency.

The Department of Children and Family Services provides brochures containing information on licensing standards to be distributed to parents. The parent will be required to sign a verification form to indicate that he or she has received this information.

It is the parent’s responsibility to keep the information in the child’s file current. Please notify the school immediately of any changes in this information.

Professional evaluations are required for admission of children with special needs.

The registration fee is non-refundable. A tuition deposit may also be required.

Medical Report

A medical report on the child (form to be provided by the school) must be filled out by a physician and returned to the school within 60 days of the date of enrollment and no earlier than 6 months prior to enrollment. This includes immunization records which must be signed by the health care professional who can verify the immunizations. It is the parent’s responsibility to keep the child’s immunization record current. The following immunizations are required by the Illinois Department of Public Health: Hepatitis B, DTaP (DPT), Hib, IPV (polio), MMR and Varicella (chicken pox). A TB skin test is required on the initial exam. If the child’s physician determines that a TB test should not be administered, a note from the physician stating this must be provided. A blood screen for lead poisoning is also required. Also recommended are a PCV for pneumonia, meningitis and bloodstream infections, and a Hepatitis A immunization.

Please notify the school when immunizations are administered so that these records may be kept current. Medical reports are valid for 2 years for children from birth to 5 years old. Parents of school age children must provide a copy of the child’s most recent school physical.

Termination of Enrollment

A parent wishing to withdraw the child from the school must give 2 weeks’ advance notice. Notice is given when the parent signs the “Notice of Termination of Enrollment” form provided by the school. When such notice is not given, Sullivan Preschool will charge 2 weeks regular contracted tuition during the notice period whether or not the child attends. If the child is withdrawn and then subsequently re-enrolls, the registration fee will be charged for re-enrollment.

If a child is absent for 2 weeks without notification by the parent of the reason for the absence, it will be assumed that the child has been withdrawn, and that space will then be made available to another child seeking enrollment. The parent will be charged the contracted tuition for this period.

Sullivan Preschool reserves the right to terminate the enrollment of a child after notice to the parent, if the child demonstrates inability to benefit from the type of care or program offered, or whose presence is detrimental to the group. No child will be dismissed unless or until every effort has been made to meet the child’s individual needs and thereby alleviate the problems through a cooperative effort on the part of the teacher, Director, and parents. If it is in the child’s best interests to terminate enrollment, the school will work cooperatively with the parent to plan for alternate care.

Enrollment may be terminated, with notice to the parent, for failure to pay the applicable charges when due, or for failure to comply with deadlines for the completion of the required medical report, including immunizations.
